Withdrawal and complaint forms

On this page, you will find document templates to help you quickly submit a withdrawal from the contract or a product complaint. Choose the appropriate form, download it, fill it out, and send it back to us in the manner described below.

How does it work?

1. Choose the correct document:

   • withdrawal form: DOWNLOAD FILE

   • complaint – price reduction or withdrawal from contract: DOWNLOAD FILE

   • complaint – price reduction after repair/replacement request: DOWNLOAD FILE

   • complaint – withdrawal from contract after repair/replacement request: DOWNLOAD FILE

   • complaint – product replacement / repair: DOWNLOAD FILE

2. Download the file to your device and fill in the required data (including full name, order number, description of the issue, and selected claim).

3. Send the completed form to us:

   • by email as an attachment to: info@ashfordandlane.com, or

   • by standard mail to: Dogheart sp. z o.o., ul. Komandorska 53a/4b, 53-342 Wrocław.

What's next?

Once we receive the form, we will acknowledge receipt and inform you of the next steps and the deadline for processing your request. In the case of withdrawal from the contract, we will instruct you on how to return the product; for complaints, we will explain the available resolution options (repair, replacement, price reduction, or refund).
